Well, about the form, you’ll need to know Japanese in order to complete the form. It’s hard to tell you here how to. Maybe ask somebody who knows Japanese, that could help. The form is actually not difficult to fill in.
And regarding the bus terminal in NGO, it’s at 3/F of 明鉄百貨. It’s a bit of walk from the Nagoya JR station.
